Submitting Supplemental Grade Report Forms (SGRF)

Undergraduate Procedure

Instructors:

  • Contact your department for a Supplemental Grade Report Form (SGRF)
  • Complete and sign the form
  • Submit the SGRF to the department offering the course

Reminders:  Exercise caution about the content of email messages.  To protect the personal privacy and confidentiality of student information do not mail grades out over insecure email.  Unencrypted email is not a secure means of transmitting information.

Department Offering the Course:

  • Review the SGRF
    • Verify the accuracy of the data
    • Review signatures
  • Retain the pink copy of the SGRF
  • Send the remaining white and yellow copies to the student’s college office
  • Verify the form has been processed by reviewing Academic History in Banner

Note:  You may need to make a xerox copy for the instructor

College Office:

  • Review the form
    • Verify accuracy of the data
    • Review signatures
  • Retain the yellow copy of the SGRF
  • Send the white copy to Academic Records, 901 W. Illinois Street, Urbana, MC-061

Graduate Procedure

Instructors:

  • Contact your department for a Supplemental Grade Report Form (SGRF)

  • Submit the SGRF to the department offering the course

  • Verify the form has been processed by reviewing Academic History in Banner

Note:  Be sure to retain the pink and yellow copies.  OAR will no longer mail these back to departments.

Important Reminders:

  • As soon as OAR has updated Banner, students can view grade corrections in Web for Student
  • Grade corrections are:
    • Recorded in Academic History
    • Not Displayed Web for Faculty
    • Reflected in DARwin once the EDW has been refreshed
  • Use the Academic History form SHACRSE – Course Summary to view official grades

NOTE:  To view details of the specific section on SHACRSE; be sure to highlight the course row, and then click on the Select icon.  You will then be taken to SHATCKN – Course Maintenance form.
