To the college office, if an undergraduate, and to the department office if a graduate student.

Signed and dated Loan Deferment forms should be delivered to the Records Service Center (Room 140), Admissions and Records Building, 901 West Illinois Street, Urbana.
How does a student update the permanent Social Security Number?
Bring a photo ID and original Social Security Card to the Records Service Center (Room 140), Admissions and Records Building, 901 West Illinois Street, Urbana. The student will then sign a Demographic Change form.
